This form is for students who are not currently hired as student employees by the University.
Supervisor: Please complete the blanks below and print this information sheet for your student employee at the time of hire. The student should bring this form to University Human Resources, located at 169 W. Union Street, to complete new hire paperwork. Upon completion of paperwork, the student will be provided with a receipt to verify that all required new hire paperwork has been completed, and return that receipt to COB Business Services. The student will then initiate STEP 3 - Workforce Assignment Set-Up, and may begin working.
The student should NOT begin until you have received verification that paperwork has been completed.
